Simplified Dual-Lumen Catheter-Facilitated Reverse Wire Technique for Markedly Angulated Collateral Channel Entry in Retrograde Chronic Total Occlusion Intervention

چکیده
The retrograde approach has significantly improved the success rates in complex coronary total occlusion (CTO) lesions. It also become predominant and important strategy CTO recanalization. However, unsuccessful crossing of collateral channels is strongest predictor failure, adverse channel morphology, including large entry angle, could reduce rate crossing. Reverse wire technique (RWT) was specifically developed for bifurcation lesions with an extremely angulated side branch, nowadays, this can be achieved by support a dual-lumen catheter (DLC). We report novel method named "simplified catheter-facilitated RWT" to facilitate markedly intervention. This new simplified making reverse bend DLC aorta instead outside guiding catheter, which feasible, effective, safe percutaneous intervention (PCI).

Retrograde Coronary Chronic Total Occlusion Intervention
Coronary chronic total occlusion (CTO) is a frequent finding in patients with coronary artery disease. It remains one of the most challenging subsets, accounting for 10-20% of all percutaneous coronary interventions (PCI). Although remarkable progress in PCI has been made, it is reasonable to state that successful recanalization of CTO represents the "last frontier" of PCI. PCI of CTOs has been...
متن کاملInitial Experience of Retrograde Wire Approach in Coronary Chronic Total Occlusion Intervention
BACKGROUND AND OBJECTIVES Retrograde wire approach has been emerged as a useful tool to enhance success rate in coronary chronic total occlusion (CTO) intervention. Therefore, we tried to report the initial experience of retrograde approach and its clinical implication on CTO intervention. SUBJECTS AND METHODS From February 2007 to July 2008, retrograde approaches were performed in 28 patient...
متن کاملSide-Branch and Coaxial Intravascular Ultrasound Guided Wire Re-Entry after Failed Retrograde Approach of Chronic Total Occlusion Intervention.
Intravascular ultrasound (IVUS) can provide valuable information during the intervention of difficult chronic total occlusion (CTO) lesion. Stumpless CTO lesions with an adjacent side branch are associated with a significantly lower success rate because the proper entry point is not always clearly identified and the guidewires easily slip into the side branch. Herein we presented a case of a st...
متن کاملRetrograde Approach for Chronic Total Occlusion Percutaneous Coronary Intervention
Large multicenter registries reporting outcomes associated with percutaneous coronary intervention (PCI) for the treatment of chronic total occlusions (CTO) have shown progressively increasing rates of overall procedural success with acceptably low complication rates in highly experienced centers. The significantly enhanced success rates of percutaneous CTO revascularization can be attributed t...
متن کاملRetrograde Percutaneous Coronary Intervention for Left Anterior Descending Chronic Total Occlusion Via an Ipsilateral Intraseptal Collateral Channel Using a Single Guiding Catheter.
Successful recanalization of chronic total occlusion (CTO) of coronary arteries has improved symptoms and mortality. In CTO cases, retrograde approach from opposite coronary artery has a better chance of procedural success. But the retrograde approach from opposite site is not always suitable. In certain CTO cases, the distal left anterior descending (LAD) artery is supplied from the intrasepta...
